Our services are intended for adults seeking evaluation and care for musculoskeletal conditions, chronic pain, sports rehabilitation, hypermobility-related disorders, and related concerns. We also provide independent medical evaluation and documentation services for eligible veterans.
Nothing on this website constitutes medical advice, diagnosis, or treatment. Information provided through our website, blog, quizzes, or chatbot is for general educational purposes only. A formal patient-provider relationship is established only through an in-person clinical visit.

Appointments are available by scheduling through our website or by contacting our office directly. We ask that you arrive on time for your scheduled appointment. Late arrivals may result in a shortened session or rescheduling at our discretion.
Cancellation Notice
We require a minimum of 24 hours' notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice may be subject to a cancellation fee. Repeated late cancellations or no-shows may result in a requirement to prepay for future appointments or discharge from the practice.
No-Show Policy
A no-show is defined as failing to attend a scheduled appointment without prior notice. No-shows may be subject to a fee and may affect your ability to schedule future appointments. We understand that emergencies occur and handle these situations on a case-by-case basis.
Rescheduling

Conrad Spine & Sport operates on a private-pay basis. Payment is due at the time of service unless a payment plan has been arranged in advance. We accept major credit cards and CareCredit financing.
We do not bill insurance directly. Upon request, we can provide a superbill or documentation that you may submit to your insurance provider for potential out-of-network reimbursement. We make no guarantee of reimbursement from any insurance carrier.
Fees for services are discussed during the initial Clarity Visit. All fees are subject to change with reasonable notice. Veterans documentation services (records review, medical evaluation, NEXUS opinion) are priced separately and are disclosed prior to the commencement of those services.
